Your real options as a foreign resident in Mexico: IMSS, private Mexican insurance, international/expat plans & out-of-pocket. What each costs & what each actually covers.

Foreign residents in Mexico have four practical healthcare options: IMSS (the public system), private Mexican insurance, international/expat-focused insurance, & pure pay-as-you-go. Most international residents in San Miguel use some combination. Here is the honest breakdown.

    1. IMSS (Public System)

IMSS (Instituto Mexicano del Seguro Social) is Mexico's public health system. Foreign residents with Temporary or Permanent Resident status can enroll.

Cost: Sliding scale by age, roughly $400 to $800 USD/year (yes, per YEAR) per person.

Pre-existing conditions: Major exclusions in the first 1 to 2 years; some conditions excluded longer or permanently.

Hospitals: IMSS facilities, in San Miguel limited; serious cases route to Celaya or León.

Reality: Many international residents enroll in IMSS as a catastrophic backstop but use private hospitals for routine care.

    2. Private Mexican Insurance

GNP, AXA, Mapfre, Inbursa, Atlas, BUPA México & MetLife México all sell residential policies to foreigners.

    Cost: Highly age-dependent. A 50-year-old couple might pay $2,000 to $4,000 USD/year for solid coverage; a 65-year-old couple $5,000 to $9,000+.

Pre-existing conditions: Usually excluded if pre-existing at enrollment. This is the most important sentence in this article. Get insurance BEFORE conditions develop.

Coverage: Strong private hospital networks in San Miguel, León, Querétaro & CDMX. Most major procedures covered.

Age cutoffs: Some insurers stop accepting new enrollees over 65 or 70. Lock in coverage earlier.

    3. International/Expat-Focused Insurance

Cigna Global, Allianz Care, BUPA Global, IMG Global, GeoBlue & similar offer plans designed for international residents.

Cost: Significantly higher than Mexican private, $4,000 to $15,000+ USD/year per person depending on age & coverage tier.

Coverage: Worldwide or regional. Some include US coverage (very expensive), some exclude US.

Pre-existing: Negotiable, sometimes covered with riders, sometimes excluded.

Best for: Residents who travel internationally often, or want US-equivalent coverage standards.

    4. Pay-as-you-go

Many residents simply pay out-of-pocket for routine care, because Mexican private medical prices are dramatically lower than US.

GP visit: $30 to $60 USD.

Specialist visit: $50 to $100 USD.

Dental cleaning: $40 to $60 USD.

    MRI: $300 to $500 USD.

Major surgery: Highly variable, but commonly 30% to 50% of US prices.

Some residents combine: cash for routine, IMSS or high-deductible private for catastrophic.

The Most Common Strategies for San Miguel Residents

Young & healthy (under 50): Private Mexican insurance + cash for routine.

Middle-aged with no preexisting (50 to 65): Private Mexican insurance with strong inpatient coverage + cash for outpatient.

Retirees with preexisting (65+): IMSS as catastrophic backstop + cash for routine + Medicare maintained in the US for trips home.

Frequent international travelers: International expat plan.

    Medicare & US Coverage

Medicare does not cover care outside the US. Most US retirees in Mexico maintain Part A (free) but drop Part B (monthly premium) since they cannot use it locally. Plans differ, talk to a Medicare advisor before unenrolling.

    San Miguel Hospitals

Hospital de la Fe, MAC Hospital, Hospital General. For complex cases, Hospital Ángeles & Hospital H+ in Querétaro (1 hour) & Hospital Aranda de la Parra in León (1.5 hours). The healthcare guide covers facilities in depth.

    The Practical Advice

    1. Get insurance before pre-existing conditions develop.

    2. Do not assume one plan does everything, most residents layer.

    3. Talk to a Mexico-experienced broker, not a US one, for Mexican coverage.
