Healthcare is one of the top three questions every prospective resident asks, & the answer in San Miguel de Allende is better than most newcomers expect.

Here is the honest, practical picture.

What healthcare is actually available in San Miguel de Allende?

Private hospitals & clinics. The two most-referenced are Hospital MAC San Miguel (a branch of the national MAC network) & Hospital de la Fe. Both handle routine inpatient, surgery, imaging, & 24-hour ER. Several specialty clinics cover dermatology, ophthalmology, dentistry, gynecology & physical therapy at a high standard.

Public hospital. Hospital General de San Miguel de Allende handles most general emergencies. Quality varies, & most international residents default to private for non-emergency care.

Specialists. A strong roster of US-trained or US-experienced specialists practice in San Miguel or rotate between San Miguel & Querétaro, including cardiology, orthopedics, oncology, endocrinology & psychiatry.

Pharmacies. Farmacias Guadalajara, Benavides & Similares all have multiple branches. Many prescriptions that require a script in the US are available over the counter at significantly lower cost.

Where do people go for major surgery or serious care?

For complex surgery, advanced oncology, & major trauma, most residents go to Querétaro (about 1 hour away) where Hospital H+, Hospital Angeles & Hospital Star Médica operate at international standards. For the most complex cases, Mexico City's Hospital ABC, Médica Sur & Centro Médico ABC Santa Fe are 3.5 hours by car.

The key practical fact: you do not need to fly to the US for serious care. The infrastructure is here, in Spanish & in English.

How do expats in San Miguel pay for healthcare?

    Four common paths:

Private Mexican insurance. GNP, AXA, MetLife, BUPA Mexico. Annual premiums for a couple in their 60s typically range $2,000 to $6,000 USD depending on coverage & deductible.

International expat health plans. Cigna Global, Allianz Care, IMG. More expensive but cover treatment back in the US or Europe.

IMSS (Mexican public system). Available to legal residents at a low annual fee (a few hundred USD). Lower-cost option, with longer waits & variable quality. Often paired with private insurance or out-of-pocket private care.

Out-of-pocket for routine care. Many residents simply pay cash for everyday care because it is so inexpensive. A doctor's visit in San Miguel often costs $25 to $50 USD. A specialist consult $50 to $100. Lab work a fraction of US prices.

The most common pattern for full-time residents: private major-medical insurance for catastrophic coverage, cash for everything else.

Does US Medicare cover healthcare in Mexico?

Important reminder for US retirees: Medicare almost never covers care in Mexico. The Social Security & retiring in Mexico guide covers this in detail.

What do most newcomers get wrong about healthcare in Mexico?

    • Underestimating the quality. Private care in Mexico is often comparable to US private care, at a fraction of the cost.

    • Overestimating the language barrier. Most specialists serving the international community here speak English or have English-speaking staff.

    • Skipping insurance for the first year. A single serious event can wipe out years of savings. Get coverage in place before you arrive or in the first weeks.

    • Forgetting about dental. Many residents fly home for big dental work then realize San Miguel does it just as well for half the price. Implants, crowns & full reconstructions are all routinely done here.

How much does healthcare in San Miguel actually cost?

In the retire at 60 guide we budget $300 to $700 USD per person per month for combined insurance & out-of-pocket. That is realistic for most couples in their 60s. Younger or healthier residents often spend less.

Is healthcare in San Miguel de Allende good enough to retire on?

Healthcare is not the reason to skip San Miguel. For most international residents, it is one of the reasons they stay. The mix of accessible local care, high-quality private hospitals an hour away, & costs that are a fraction of US prices makes it possible to live well & age in place here.
